Insurance Information

MMA participates in most area health plans. Patients are responsible for knowing plan benefits, co-payments, non-covered services and restrictions and are responsible for co-payments and non-covered services at time of visit.

If you do not have health insurance or are self pay, you can request a Good Faith estimate before you receive care.

What to Bring to Your Appointment

Please bring to your appointment the following:

  • All your medicines in the original containers, including any vitamins and herbal supplements you may take
  • Your insurance and/or Medicare card
  • Any medical records you have from your primary physician
  • A government issued photo ID, i.e., driver’s license
  • Motor Vehicle Accident paperwork, if applicable
  • Workman’s Compensation paperwork, if applicable

What to Expect During Your Appointment

Upon arrival for your appointment, please check in with the receptionist. We know your time is valuable and make every effort to keep appointments as close to the scheduled time as possible. However, in healthcare, emergencies sometimes arise. On the rare occasion that your appointment is delayed, we will keep you informed about the delay.

Your first visit will include measurement of height and weight and a review of medications by a Medical Office Assistant. You will then see the physician, who will interview you about your medical history and current condition.

After the exam, the physician will discuss his/her findings and recommendations with you. A nurse will provide further education as needed and review the physician’s recommendations.

How to Cancel Your Appointment

Should you need to cancel your appointment for any reason, we request that you call 24 hours in advance whenever possible. If you cannot call at least 24 hours ahead, please call as soon as you know you need to cancel. Chances are, another patient can use your appointment time.

We look forward to meeting you and making your visit to Murray Medical Associates as pleasant as it can be.
